Sealed proposals, subject to the conditions contained herein, will be received by the City of Opelika, Alabama, until 2:00 p.m., local time, Tuesday, October 20, 2026, and then publicly opened and read aloud at the City of Opelika Information Technology Training Room at 204 S. 7th St, Opelika, Alabama from qualified providers to provide:

Employee Health and Wellness Services 

All interested parties are invited to attend.  Only proposals demonstrating compliance with all minimum requirements will be considered. Failure to meet any single requirement will result in immediate disqualification as non-responsive, and the proposal will not be evaluated.

Minimum qualifications:

1. Must have a verifiable record of managing and operating at least one (1) health center/clinic for a municipality or other governmental entity for a period of at least five (5) consecutive years, providing comprehensive wellness or occupational health services comparable in scope and scale to those sought by the City of Opelika.

2. Must have a record of no less than five (5) years of satisfactory contractual performance demonstrated through the provision of no less than three (3) references.

3. Must be able to provide all services at the City’s existing clinic facility located at 105 N 10th St, Opelika, Alabama 36801.

4. Proposer must maintain full compliance with all Health Insurance Portability and Accountability Act (HIPAA) regulations and all applicable local, state, and federal rules, regulations and laws.

5.Proposer must agree to release, indemnify, hold harmless, and defend the City from and against all liabilities arising out of any act or omission of the Proposer, its agents, or employees, and must maintain the minimum insurance limits specified in this RFP.

All proposers shall possess all licenses and/or permits required by applicable laws, rules, or regulations for the performance of the work.

The proposal must be submitted and organized in the manner given in the request for proposal by the City of Opelika.  All information in the proposal must be completed by the submitter for the proposal to be accepted.
